Accessibility Feedback Form

An accessibility feedback form gives website visitors a way to report difficulties using its content or functions.

An accessibility feedback form gives website visitors a way to report difficulties using its content or functions. Someone might describe a keyboard navigation obstacle, an unreadable document, or a form field that a screen reader does not announce properly. A small service business should provide a usable contact alternative in case the feedback form itself is inaccessible. The process should make clear what information helps investigate an issue and who will respond, without demanding unnecessary medical details. A feedback form is different from an accessibility statement, which explains the site's commitments and known limitations. Receiving reports is useful, but it does not by itself establish accessibility compliance.
