5 Whys Analysis

5 Whys analysis is a root-cause investigation technique that repeatedly asks why a problem occurred to uncover a contributing process failure.

The 5 Whys is a simple questioning method for investigating why an operational problem happened. Instead of fixing only the visible symptom, a team asks why the problem occurred, then continues questioning each answer until it identifies a cause it can address. Five is a guide, not a mandatory number of questions.

For digital services, the technique can clarify why a website lead went unanswered, a form failed, or an update created a recurring error. A missed inquiry may trace back to an unmonitored inbox, an expired notification integration, or an unclear staff handoff. Recording evidence beside each answer helps prevent the group from mistaking assumptions for facts.

The method works best for relatively bounded problems with a traceable chain of causes. Complex incidents can have several interacting causes and require logs, measurements, interviews, or other root-cause tools. It is not an excuse to blame the individual at the end of the chain. Useful corrective action changes the process, verifies the fix, and checks whether the problem returns.

A small agency might pair a 5 Whys worksheet with incident timelines and a named owner for each corrective action.
